Reducing Risk through Mobile Integrated Healthcare

In 2012, the Chief of EMS, now Fire Chief for the Tulsa Fire Department began addressing 9-1-1 high utilizers to see what the fire department could do to better assist the individual and reduce 9-1-1 response. Fast forward to 2024 and TFD now has a formal MIH program consisting of 5 specific programs addressing community specific needs through Social Determinants of Health (SDoH). TFD utilizes software to provide real time documentation and interagency communications of care plans with community partners.
We will discuss the 5 programs, staffing types, objectives, and outcomes. The 5 programs are TFD CARES, Community Response Team, Alternate Response Teams 1 and 2, and the Opiate Response program. All 5 programs utilize staffing models with everything from sworn firefighters to case managers, peer recovery support specialist, to licensed clinicians. Each team has specific purposes and defined response type. All teams utilize Julota to provide HIPPA, 42 CFR Part 2, and CJIS compliant software to ensure client connection with services. By addressing Social Determinants of Health we have been able improve the delivery of community healthcare.
In this presentation, we will take participants on a journey to understand how our approach through SDoH has helped to answer root cause issues affecting some of our most vulnerable populations that are calling 9-1-1. Participants will get a glimpse out TFD's approach, our MIH teams, how we leverage data sharing agreements to improve continuum of care, reduce firefighter injuries, reduce 9-1-1 utilization, and improve health and well-being in our community.
The presentation will provide opportunities for the participants to interact through real time surveys and input. This is accomplished utilizing multimeter in the presentation.
Learning Objectives:
Upon completion, the participants will be able to articulate how Mobile Integrated Healthcare (MIH) can reduce community risk and improve fire department operations by addressing specific community risk.
Upon completion, the participants will be able to define how facilitated software workflows can improve interagency communications and continuum of care for clients.
